Binary verdict (is_safe ∈ {0, 1}) + confidence score + reason string. Rejected facts persist with quarantine_status='sanity_rejected' (M1 lossless invariant preserved).

- 3-stage rollout: v0 rule-based pattern matcher (live default), v1 DeBERTa-v3-base fine-tune on 50K-row corpus once data accumulates, v2 nightly retrain on user corrections.
- Recall ≥ 0.95 at FPR ≤ 0.5% acceptance gate.
